It is time to renew your CCDTC membership. The new club year runs from October 1, 2011 through September 30, 2012. Here’s some information that may help you when filling out the form:

• 1) We need up-to-date contact information on everyone in the club, so we need every member to return the renewal form with their check. Please do not send your check without the signed renewal form. Lifetime Members also should return the form if they wish to be listed on the membership roster and in the directory. We are requesting that Lifetime Members who are active in the club pay the cleaning fee.
  
• 2) Please initial the section that states that any dogs you bring to the club are healthy and appropriately vaccinated. You do not need to send in copies of your dogs’ shot records.
  
• 3) The last day to apply volunteer hours for the 2010-11 year is September 30, 2011. Members must add $15.00 to their regular dues for every required service hour not worked. Remember, single members need 16 hours, families need 22. If you joined the club this year, your hours are prorated—contact Charlene Reese at charlenereese72@yahool.com or call 513-271-6122 if you are not sure how many you need to work.
  
• 4) If you joined within the last few months and paid a full year’s dues (instead of the prorated amount), you may pay the prorated amount for the coming year. Contact Charlene Reese if you have questions about how much you should pay.
  
• 5) A release statement is included on this year’s renewal form. This should be signed by every adult member of the club.
  
• 6) Inactive members can change their status back to Active with Board approval. Active members who are considering leaving the club should first consider applying for Inactive status if they think there is a chance that they will become active again within a few years. Contact Monica Holmback if you have questions.
  
• 7) Please return your membership renewal to the address on the form by October 1, 2011, or as soon as possible. FORMS AND PAYMENTS LEFT AT THE CLUB MAY NOT BE CORRECTLY CREDITED TO YOUR ACCOUNT. Dues and fees not received by the Annual Meeting in November will be assessed an additional late charge of $10.00. Any membership not renewed by December 31, 2011 will be terminated.
  
• 8) Please remember to include the appropriate building cleaning fee as noted on the form. One check can be sent to cover all fees.

To remain a member in good standing, all fees must be paid.
